DUMB Module decoder

Adds decoding support for several sequenced music formats: MOD/MDZ, S3M/S3Z, STM/STZ, IT/ITZ, XM/XMZ, PTM/PTZ, MTM/MTZ, 669, PSM, UMX, AM/J2B, DSM, AMF, and MO3.
Version 0.9.9.14 highlights: Updated zlib to version 1.2.4.
Version 0.9.9.15 highlights: Amended preferences dialog initialization handler.
Version 0.9.9.16 highlights: Fixed tracks terminating correctly when preferences are configured for infinite looping and caller requests no looping, like converter or ReplayGain scanner. Optimized opening and decoder initialization process to only parse most opened modules once instead of twice or more.
Version 0.9.9.17 highlights: Implemented a channel mute control dialog.
Version 0.9.9.18 highlights: Fixed channels unmuting when seeking backwards.
Version 0.9.9.19 highlights: Fixed channel muting when seeking, restarting playback, and switching files.
