AdPlug decoder

Adds decoding support for all Adlib FM chip formats supported by the AdPlug library.
Version 1.2 highlights: Implemented support for Harekiet's fast and accurate OPL emulator core from the latest Dosbox.
Version 1.21 highlights: Disabled the Westwood ADL format support until it can be fixed.
Version 1.3 highlights: Updated Harekiet's OPL emulator core to latest, and doubled its output volume to match the other cores.
Version 1.31 highlights: Fixed input in case the AdPlug database file is missing.
Version 1.32 highlights: Added some debug logging.
